A Message from Our Chief Nursing Officer, Amanda Snyder, BSN, RN

At the heart of everything we do is a simple principle: exceptional care at home must be nurse-led and physician-guided.

While our team leads the day-to-day clinical care in the home, we work hand-in-hand with physicians to ensure every care plan reflects their expertise, guidance, and medical oversight. We follow their clinical direction closely, communicate proactively about changes in condition, and coordinate care so that treatment plans translate seamlessly from the clinical setting to the home environment.

Because we care for clients in their home environment, which may also be facility, our nurses and caregivers often have the unique ability to observe subtle, and sometimes not so subtle, changes in a client’s condition that may otherwise go unnoticed between medical visits. In many ways, our team serves as the physician’s eyes and ears in the home setting, communicating important observations, changes, and concerns so physicians have the timely information they need to guide care. This allows us to advocate on behalf of our clients while ensuring their physician’s clinical expertise continues to shape the care plan.

This partnership is what allows us to support recovery, maintain stability, and help our clients live safely, comfortably and with dignity at home. Simply put, excellent care at home depends on strong physician collaboration, and we are deeply grateful for the trust and partnership our referring and treating physicians provide every day.

As we approach National Physicians Day later this March, we are reminded of the important role physicians play in caring for our communities. The day is meant to recognize the dedication, skill, and compassion physicians bring to their patients each day and to acknowledge the impact they make across the entire healthcare system.

To celebrate the physicians who partner with us in caring for clients at home, we are excited to extend a heartfelt invitation to our partner and referring physicians to join us for our first ever end-of-season appreciation reception on April 9 at theMartin Arts. It will be a wonderful opportunity to connect, celebrate our shared commitment to patient care, and thank you personally for the role you play in making exceptional care at home possible.
